Structured issue forms for bug reports and feature requests: environment, steps, expected/actual behaviour and logs for bugs; problem, proposal and alternatives for features. Both ask reporters to search existing issues first and keep one problem per issue; blank issues are disabled and the latest release is linked from the chooser. Forms do not require a particular language. Refs #860
